Is it possible to restrict what a Renderer device can show based on parental ratings like R rated Movies and Explicit Language in music?

Not at the moment - Mezzmo doesn't support parental rating. We do plan to add this feature, so once it's implement then it'll be possible to restrict playlists to certain devices to certain parental ratings. For the time being, you can still restrict certain playlists using the Playlist Sharing Permissions settings (right-click on a playlist in the tree to access this).

Now with Blu-Ray players getting so cheap it's getting to a point that most people will start putting them in (almost) every room. I know I have just put one in the kids' toy room.

Rather than having Mezzmo use parental ratings is it possible to have a PIN set on certain directories? That way all directories can still be seen by all the Blu-Ray players in every room but the videos (or music) that contain more mature product will need a PIN before it can be accessed.

For example I have a Blu-Ray player in our family room. I don't want to block everything by IP address because there are times I am going to watch the blocked content, but I don't want my 7 year old being able to access it. By entering a PIN it will allow me to watch my True Blood episodes (for example).

Unfortunately, DLNA does not provide any means for entering numbers/letters for a PIN to access a folder. We do understand your situation and we agree it would be great if Mezzmo could let you access restricted folders with some means of password/PIN access, etc. We'll think about it here more and try to come with something.
