It appears that the Mezzmo app on the Fire TV (and presumably Fire Stick) is limiting audio playback to 2 Channel.

Is this is a limitation of the app or the Fire TV appliance?

The Mezzmo app on the Fire TV decodes the audio to 2 channel for playback, this is a limitation of the app. For surround sound support you can try using the Kodi app on the Fire TV with the Mezzmo plugin.
