What settings do I need to have in the profile to enable playing as the file transcodes?

The format you are transcoding to needs to support playback before the transcoding is complete. mp4, avi and mkv have an index at the end of the file that is not available until the transcoding is complete. Some devices will play mkv without the index. mpegts is the best format for transcoding as it does not have an index.
Ahhhh, thats probably what it is - I was watching mp4s more than mkvs previously, but lately more have been mkvs.
I'll do some more research...
