The issue could be related to the source format of the file. What container are you using (i.e. MKV, MP4, M2TS etc..) and have you tried different ones to see if the issue is specific to a particular format ? You might want to check what container format that transcoding was using and see if you have some files already in that format and whether FF/RW works with them with transcoding disabled. That would confirm whether this is related to a specific container format.
